On this page you will find links to download the forms necessary to keep an accurate record of changes made toyour registrtion status.You can download these RTF (Rich Text Format) documents and fill them out on your favorite word processing software.
The deadline for changing grading option or changing registration status to V (Visitor) is the end of the fourth week of each quarter. The deadline for withdrawal from courses is the sixth week of the quarter. After these deadlines, you will need to fill out a Petition to make changes.
Admission Form (PDF). This is the form you must send in to permit the Registrar to establish a permanent record of your academic achievements.
Change of Status Form (RTF). This is the form you need to change your registration status from regular credit registration to Visitor (V) or Whithdrawn (W). To drop a class, you must file this form to change your status to "W" for Withdrawn. (See Instructional Policies for more information on registration status.)
Change of Grading Option (RTF). This is the form for changing from the numerical grade option to the P/N option or vice versa.
Petition Form (RTF). This is the form you need to petition the Registrar to change your status or grading option after the deadline.
